There's many different versions, but most of the time the ritual done does not involve tearing the hooks through. In modern non-native ritual in the BDSM/piercing/bodymod/modern tribal/burner kind of culture when this kind of pulling piercing is done in the chest region it's called a 'heart pull'. I've been to workshops/rituals for this and other forms of hook suspension, but not participated directly as someone being pierced with the hooks and only there as support.

People who are inexperienced with this type of transcendent pain will automatically have a negative, queasy kind of reaction. However I can assure you the vast majority of participants have a literally mind blowing time. I have seen varied responses, often orgasmic in nature not just physical but spiritual, to rites of pain. Our society only teaches a one sided relationship to pain, where I believe our natural relationship with pain is truly far more complex.
